❋ Credit Card on File-Cancellation/Late/No-showWe require our patients to always have a current updated card on file- this is company policy and requirement to be a patient here at Brandy Marie Family & Aesthetic Care.
It will be used to pay last minute cancelation, late arrival, & no-show fees
❋ Appointment Cancellation & No-Show PolicyLife happens! Please call at least 24 hrs before your appointment to cancel or reschedule and avoid a $50 fee. Missed visits (no‑shows) without notice will be charged $100. We appreciate your cooperation and can assist with rescheduling.
To ensure appointment availability for all patients and maintain the financial stability of the practice, we require timely notice for appointment changes. Missed or late‑cancelled appointments result in lost clinical time and reduced access for other patients.
❋ Financial Responsibility & Collection Policy
(Utah H.B. 128)
After insurance processes your claim, you’ll receive a notice with your balance, due date, and a warning that unpaid amounts may be sent to collections or reported to credit bureaus. Payment is due no sooner than 45 days from notice (60 days for Medicare). Most patients have a card on file, which may be charged for unpaid balances or no‑show/cancellation fees. Authorization remains active until you revoke it in writing.
❋ Self-Pay Financial Responsibility & Collection Policy
For patients who are paying for services directly (self‑pay), payment is due in full at the time of service unless other arrangements are made in advance. If your account becomes past‑due and satisfactory payment arrangements are not made, it may be referred to an external collection agency. You will be responsible for any associated fees, including a collection fee of up to 35% of the outstanding balance and reasonable costs of collection, such as attorney’s fees. By providing your contact information, you authorize Brandy Marie Family & Aesthetic Care and its designated collection agency to contact you by telephone (including wireless numbers), text or email for account‑related purposes.
